The convenience of clinical plus nutritional interventions in gastroenterological patients

Abstract

The advantages of the multiprofessional approach for diagnosis and treatment of hospitalized patients is becoming evident and mandatory. The result of this multiprofessional interaction is analysed in the present article under the view of nutritional therapy. Two mala adults patients suffering from chronic diarrhea of two distinct etiologias: traumatic retite and lymphoproliferative disease were both severely malnourished at the admittance. The nutritional assessment (dietary intake, anthropometry and blood biochemistry) was done weekly throughout the hospitalization. Dietary intervention consisted of either oral (obstipant), enteral and/or parenteral supports. Both patients reacted well to diets decreasing the defecation rate and doubling their serum leveis of free tryptophan. However the nutritional improvement was sustained only in the case where the basic disease (lymphoproliferative disease) was conveniently treated. ln the other case the anal-sphincter reconslruclion was not undertaken, and probably because of that lhe improvemenl of nutritional indicalors was transienl and ceased soon after the dielary support. So it was evidenl the inability of diel therapy in keeping atone the nutritional state of the patienl without lhe adequale clinical-surgical procedure.
