Obturator internus muscle abscess simulating hip septic arthritis

Radionuclide imaging and CT scan are, nowadays, the most efficient way to diagnose obturator internus muscle abscess. The patient, a 15-year-old boy, presented a 10-day history of pelvic pain and limping. The hip physical examination showed normal mobility. Hematological studies revealed hvperleukocytosis and an abnormal ESR, Pelvic x-ray and ultrasound appeared to be normal. Radionuclide imaging showed a small probability of septic arthritis and the possibility QI a pelvic abscess. The first CT scan revealed a public cortical abnormality, a lack Q/ definition and a diffuse swelling of the right obturator internus, right obturator externus, pectinate, right long adductor and right short adductor. After the use of Oxacilyn for 21 days, a new CT scan showed a significant local improvement. CT scan is the most efficient procedure not only to diagnose but also to evidence the remission of the abscess.
