Abstract

The treatment of pheochromocytoma consists of surgical excision of the catecholamine-producing tumor, in the adrenal or extra-adrenal location. The publication of the first cases of laparoscopic adrenalectomy in 1992 caused a revolution in the treatment of adrenal injuries. Since then, laparoscopic adrenalectomy has been widely performed and is currently the treatment of choice for benign adrenal tumors, whether functioning or not. With the aim of evaluating the role of laparoscopic surgery in the treatment of pheochromocytoma, a review of the medical literature was carried out from the year 1992 onwards. The performance of a significant number of laparoscopic surgeries in the treatment of pheochromocytoma made it possible to verify that metabolic changes and hemodynamics are similar to those observed with open surgery, without an increase in risk for laparoscopic surgery, in addition to open surgery being associated with a greater release of catecholamines than laparoscopic surgery. Laparoscopic surgery for pheochromocytoma also proved to be effective and safe when performed in cases of paragangliomas, in bilateral tumors, in retroperitoneoscopic surgery and even in laparoscopic partial adrenalectomy of small tumors in patients with familial pheochromocytoma. When compared to open surgery, laparoscopic adrenalectomy presented the advantages of a shorter period of hospitalization and convalescence, lower morbidity, the same effectiveness, safety and better aesthetic results. Once the learning curve has been overcome, both techniques can now be performed with the same amount of time. Analysis of the results demonstrated the efficacy and safety of laparoscopy in the treatment of pheochromocytoma.
