Large ovarian mucinous cystadenofibroma with focal epithelial atypia and proliferation in pregnancy

Objective
Adnexal masses are rare during pregnancy, with an estimated incidence of 1% to 6%. Mucinous cystadenoma, typically unilateral, can reach large proportions due to its indolent course. The objective of this study was to report the case of a pregnant woman with a large ovarian mucinous cystadenoma exhibiting atypia and focal epithelial proliferation, surgically treated during pregnancy.
Method
Case report and literature review.
Results
A 33-year-old primigravida presented with a 32 cm adnexal mass and underwent laparoscopic unilateral
oophorectomy at 16 weeks of gestation. The diagnosis was cystadenofibroma with atypia and focal epithelial proliferation. The patient progressed to full-term delivery and showed no recurrence of the disease after 3 years and 6 months post-surgery. A review of the literature identified seven studies describing large mucinous cystadenomas
in pregnant women, all managed via laparotomy.
Conclusion
Despite the significant size of the cyst, laparoscopic management was feasible during pregnancy, without
dissemination of cystic fluid into the abdominal cavity. While a favorable prognosis is expected for these patients, there is limited data in the literature regarding whether atypia or focal epithelial proliferation in mucinous cystadenomas increases the risk of disease recurrence.
